Making an enquiry
The contact form provides fields for your name, email address, and message. Your enquiry is sent to the practice by email so I can respond.
Please use the form for a brief question about services or appointments. Avoid including detailed clinical history, identification documents, payment information, or unnecessary information about another person. You can also contact me by telephone at 306-570-5024.
Appointment booking and video appointments
Online appointment booking is provided through Calendly. Video appointments use Doxy.me. These services are operated by separate providers, with their own privacy information:

Website services
This website uses Google Fonts to display type, Google reCAPTCHA to help identify automated form submissions, and Cloudflare for website security.
